Enrollment Instructions

  1. You can complete the New Student enrollment/Re-enrollment forms online and save to your computer. A form is needed for each child that is being enrolled.  
  2. Print the completed application and bring it with you to school to finish the enrollment process
  3. You can submit the completed application by hitting the Submit button on the form.
  4. Bring to the school a copy of the student(s) birth certificate, immunization record, social security card, and proof of residence (a document with your name and address on it.)
  5. You can sign and complete the application at the school.
